NAME TAG ORDER FORM
Each name tag has the EAA logo, measures 3" by 1" and may have up to 3 lines. You may wish to include your name, chapter number, city and state, or your airplane "N" number. For new chapter members, the cost is paid as part of the one-time $8.00 initiation fee. Cost is $7.00 each for current chapter members. Make checks payable to EAA Chapter 186.
